People dying after falling in open pit

There are news reports of people dying after falling in open pits left on roads without proper fencing or barricades. People are reported to be dying after falling in manholes left without covers. Children are being infected with HIV when transfusion of infected unscreened blood is being done in government hospitals in remote parts of the country. These are all preventable. Is it not the responsibility of officers to see to it that such things do not happen? When new recruits join the services, the motive should not be self aggrandisement and self promotion laced with hypocrisy, but public service. Only a change in mindset and a spirit of service can change things.
